you have medical insurance, the office is anxious to help
you receive your maximum allowable benefits. We accept
a variety of different policies. You must realize, however,
that: |

Your
insurance is a contract between you, your employer,
and/or the insurance company. This office is not
a party to the contract. |
| 2.

Insurance companies pay according to what they set
as a contracted fee or as usual, customary, and
reasonable. Most insurance companies consider the
fees of this office usual, customary, and reasonable.
These insurance payments may not always be the same
as Dr. Pat Solis’ charges. If your insurance
company on a contract basis for a contracted fee
makes payment, you will only be responsible for
co-payments, co-insurance, and/or deductible amounts
of this fee. |

all services are a covered benefit in all contracts.
Some insurance companies arbitrarily select certain
services they will not cover. |
