This
office functions as a Primary Care Office
providing Minor Medical Services for optimal
health. Referrals are made to medical specialists
when they are needed. Appointments are made
for patients upon request when they are referred
to another doctor. |

Appointments |
|
Patients
are scheduled by appointment. On your first visit,
a permanent record will be initiated. You will be
asked to complete a registration form. Try to arrive
a few minutes before your scheduled appointment.

Canceling
An Appointment |
|
If
you are unable to keep your appointment or if you
are going to be late, please call the office as
soon as possible. This courtesy allows the office
to be of service to other patients. The answering
service will relay messages to the staff in case
the office is closed. |
|
Office
Visits |
|
In
an effort to keep medical costs down, it is asked
that you pay for all office visits, lab, etc. at
the time services are rendered. You may use your
personal check or cash.
Charges for office visits and surgery are determined
by the time spent and vary with the severity of
the problem. These charges are competitive with
other area specialists. Fees reflect the level
of care you will receive and the degree of training
by the physician and staff.

Medical
Records and Confidentiality |
|
All
your medical records are confidential. Your medical
records or any part(s) of them will ONLY be released
if you give written authorization. Information regarding
your condition will not be given to you employer,
friends, or relatives without your permission. The
only exception to this is when required by law. |

|
Telephone
Calls |
|
Feel
free to call the office if you have any questions
regarding your condition, treatment, or medications.
The medical secretary or nurse will help you or
will check with Dr. Solis and get back with you.
Many diseases have similar symptoms; so if you
have any changes or develop a new problem, please
understand the possible need for an office visit.
The
telephone is answered by the staff or answering
service 24 hours a day including weekends and
holidays.

Emergency
Care |
|
If
you have an emergency, please call the office as
soon as possible. After office hours, the answering
service will relay emergency patients calls to Dr.
Solis or to the physician who is taking his calls.
In an emergency crisis, it is best for you to
go to the nearest emergency room where the physician
on duty will begin treatment.

Insurance |
|
If
you have medical insurance, the office is anxious
to help you receive your maximum allowable benefits.
We accept a variety of different policies.
You must realize, however, that: |

Your
insurance is a contract between you, your
employer, and/or the insurance company. This
office is not a party to the contract. |
|
|
|
Insurance
companies pay according to what they set as
a contracted fee or as usual, customary, and
reasonable. Most insurance companies consider
the fees of this office usual, customary,
and reasonable. These insurance payments may
not always be the same as Dr. Pat Solis’
charges. If your insurance company on a contract
basis for a contracted fee makes payment,
you will only be responsible for co-payments,
co-insurance, and/or deductible amounts of
this fee. |

Not
all services are a covered benefit in all
contracts. Some insurance companies arbitrarily
select certain services they will not cover.
