Patients are scheduled by appointment. On your first visit, a permanent record will be initiated. You will be asked to complete a registration form or you may print and fill in the Patient Registration Form prior to coming in for your appointment. Be sure to bring this Patient Registration Form with you on the day of your appointment and to arrive a few minutes early.

Canceling An Appointment
 
If you are unable to keep your appointment or if you are going to be late, please call the office as soon as possible. This courtesy allows the office to be of service to other patients. The answering service will relay messages to the staff in case the office is closed.
